TMSS-156: used #ifdef DO_COMPILE_OTDB_AND_PVSS_CODE everywhere, so that we have a compilable and runnable MACScheduler that polls TMSS

Task #7593: autogen creates the protocol files now in the namespace XXX_Protocol. Modified the sources to look in these namespaces.

Jan David agreed it makes sense, although some software covered is not
currently maintained by the RO software team.
All replacements happen in comments, so this is safe. (viewed svn diff | grep ... by hand)

Task #3250: Include the new ApplCommon/LofarDirs.h header file when using any of the LOFAR_*_LOCATION preprocessor variables.

Task #3048: ObsClaimer does not write beamlet list in PVSS anymore since this list is different on every station.

Bug 1284: The availability of the receivers is only known at the stations, so this info is moved from
'Observation' to 'StnObservation' in PVSS.
When the scripts in PVSS are updated the code in the ObsClaimer.cc should be removed.

The ringname only depends on the first two letters of the name.
