[RAP-210] If no patch exists, group all sources into a single patch

Frits Sweijen requested to merge RAP-210 into master

If an input sky model has no patches, transferring the patches from tessellation will fail. This MR groups all sources in a single patch before attempting to transfer patches in that case.
