Update header format and vis-upgrader

Duncan Kampert requested to merge freq-header-change into master

vis-upgrader is updated to convert old vis files to new vis file header information rather than the previous intermediate format. The intermediate format is now also accept by vis-upgrader. The add-subband helper program is no longer needed and removed.
