Apply lm shift in wtiling

Bas van der Tol requested to merge apply-lm-shift-in-wtiling-2 into master

The w-tiling code has been extended to take the shift parameter into account. The computation of the support of the w-kernels has also been updated to include the shift. Documentation on the derivation of the support is in the doc sub-directory.

DPPPs FacetPredict can now use idg with w-tiling enabled.
