Document procedure for upgrading ESAP databases in-place

A new version of ESAP may be accompanied with updates to the database schema. ESAP should be shipped with migrations to update existing databases, enabling deployments to be upgraded to the new version without re-creating the database from scratch. We need to document how this works.
